Description

Session Description

The need for stable housing, healthy food, and good jobs are universal in our communities, and yet people everywhere increasingly struggle to meet these basic needs. This session will look at various sources of data, including community health assessments and patient surveys, for insights into the common social needs people face today and share strategies for addressing them. Programs include a Food is Medicine Center of Excellence to propel nutrition-based treatments and interventions in heath care; medical-legal partnerships to protect people from losing their homes unjustly, and a very specific strategy to increase the mental health workforce to meet growing demand for services, particularly in vulnerable communities, by helping mental health professionals complete licensure.

Session Takeaways

  • Develop an understanding of prevalent health and social needs in our communities.
  • Discover health plan strategies for addressing common social factors at the individual and community level.
  • Learn how data analytics can help define priorities and design interventions.

Ideal Audience Description

This session is ideal for a wide variety of stakeholders, including payers, health care providers, social care providers, and beneficiaries interested in designing and implementing programs to address prevalent health needs at the individual and community levels.
