Institute for H.O.P.E.TM at The MetroHealth System expands the traditional medical model of care to include addressing the social drivers of health, yielding greater access to health care and improvements in health outcomes. With a focus on improving healthy equity for our most vulnerable communities, SDOH screening data is used to determine intervention opportunities through research, evaluation, resources, and programs. During this session, the presenters will provide several examples of programs that address social isolation, food insecurity, and housing quality and stability, through mutually beneficial partnerships with community-based organizations.
