Session Description
U.S. health disparities in asthma affect Black, Hispanic, American Indian, and Alaska Native communities. AAFA’s HEAL program provides specialized care, self-management education, and a home environmental assessment to enhance asthma knowledge and confidence. Community-driven programs, like HEAL, can effectively reduce asthma deaths and illnesses. The session will highlight the program development, evaluation, and lessons learned from this community health pilot. The AAFA-Michigan, Chapter’s approach to addressing asthma control by combining nutrition and physical activity opportunities within their program will be explored.

CHWs play a pivotal role in improving asthma control among participants. Addressing SODH needs such as food security necessitates creative approaches and partnerships to comprehensively address asthma-related challenges. Enrollment and program results take time, particularly for adults who may not prioritize asthma care. Patience and flexible funding are crucial, with several months required for engagement.
